(Background and Purpose) Population ageing has become a core urban policy agenda worldwide, and Korea is on the verge of entering a super-aged society. In response, the World Health Organization (WHO) proposed the Age-Friendly City framework with eight core domains and launched the Global Network for Age-friendly Cities and Communities (GNAFCC). This study compares the policy operations and administrative foundations of early-joining Korean GNAFCC cities—Seoul, Suwon, Busan, Jeongeup, and Jeju—to examine the relationship between policy maturity and local characteristics and to suggest directions for future development. (Method) The study was conducted focusing on literature analysis and policy document analysis by setting the WHO's eight areas (external environment, transportation, housing, social participation, respect and inclusion, civic participation and employment, information and communication, community support, and health services) as an analysis framework. Each city's ordinances, mid- to long-term plans, annual action plans, statistical annals, and public policy data were reviewed, and the institutionalization continuum leading to ordinance-organizational placement-participatory governance-planning-evaluation and its implementation characteristics were compared qualitatively and quantitatively. (Results) Seoul and Suwon exhibit higher policy maturity and sustainability, supported by dedicated functions within government, multi-year implementation plans, and institutionalized citizen participation. Busan has developed a metropolitan, integrated model linking transportation, health, and housing, commensurate with its scale. Jeongeup and Jeju have strengths in community-based, place-based services typical of rural or mixed urban–rural settings; furthermore, recent ordinance updates in Jeongeup (2024) and Jeju (2025) strengthened the legal basis for committees, monitoring, and planning, indicating movement toward partial institutionalization. Across the WHO domains, implementation tended to be stronger in community support & health services, transportation, and social participation, while civic participation & employment and communication & information showed greater room for improvement. These patterns suggest that city size, administrative capacity, fiscal resources, and community network strength are decisive determinants of age-friendly policy maturity. (Conclusions) Korean cities commonly adopt the WHO eight-domain structure but follow differentiated development pathways depending on urban scale, functional territories, and administrative capability. To advance age-friendly policies, local governments should: △ enhance legal and institutional coherence; △ adopt typology-based, city-specific strategies—cross-domain packages for large cities and accessibility/coordination strengthening for smaller or mixed urban–rural cities; △ institutionalize meaningful participation of older adults; △ improve digital inclusion and information accessibility; and △ formalize performance evaluation and feedback cycles. Ultimately, age-friendly policy should evolve from an extension of social welfare into an inclusive urban innovation strategy that reorganizes urban structures and social relations. This study provides comparative evidence to support that transition.
