This study examined changes in residential falls and associated risk factors among adults aged 65 years and older hospitalized for injuries before and after the COVID-19 pandemic. Data were obtained from the National Hospital Discharge In-depth Injury Survey. Individuals hospitalized during 2017–2019 and 2021–2023 were included, with 2020 excluded due to changes in healthcare utilization. Complex-sample cross-tabulation and logistic regression analyses were performed. The proportion of residential falls increased from 26.3% to 31.1%. Female sex, age ≥75 years, residential injury location, activities of daily living, and surgery were significantly associated with increased fall risk. However, the post-pandemic period was not significantly associated with fall risk, and no significant interaction between residential setting and the pandemic period was observed. These findings suggest that residential fall risk is primarily influenced by individual vulnerability, environmental context, and activity-related factors rather than the pandemic period. Strengthening home safety and targeted prevention strategies for high-risk older adults are needed.
