LASIK 후 발생한 각막염의 임상적 치험 6예

레이저각막절삭가공성형술(LASIK)은 비교적 안전하고 성공율이 높은 근시교정술로 수술 후에 각막염 발생은 매우 드문 합병증으로 알려져 있다. 저자들은 LASIK후에 발생한 각막염 환자 6명, 10안을 경험하고 그 위험인자와 치료방법에 대해 알아보고자 하였다. 4명의 환자 6안에서 안검염과 연관되었는데 이중 3명에서 안검의 세균배양검사 상 S. epidermidis가 분리되었고 2명의 환자 4안에서는 건성안과 연관된 재발성 표층점상각막병증을 보였다. 저자들은 S. epidermidis가 분리된 난치성의 각막염 환자와 비전형적 헤르페스 각막염환자에서는 양막이식으로, 광범위 층판각막염을 보인 환자는 스테로이드 점안치료로, 표층점상각막병증을 보인 환자는 누점마개와 혈청치료로 효과적으로 치료하였다. LASIK에 의해서 만들어진 각막편과 엑시머레이저 조사에 의한 각막간질 및 신경의 손상은 정상적인 각막의 창상 치유가 이루어지지 않게 하고 각막의 방어기전에 손상을 입히기 때문에 수술 후 각막염의 위험이 항상 따른다. 저자들이 경험한 LASIK후 발생한 각막염의 위험인자는 안검염 및 건성안이었다.<BR> 이상의 결과에서 LASIK시술 전에는 반드시 위험인자들에 대한 신중한 검사가 이루어 져야하고 각막염 발생시에는 적절한 치료가 필요하다고 생각된다(한안지 41:2732~2740, 2000).

LASIK is considered as a safe method of operation with high success rate and keratitis is a rare complication. This study aimed to report ten eyes of 6 patients with vision threatening keratitis and its risk factors following LASIK. Six eyes of 4 patients were concerned to severe blepharitis and 3 patients of them cultured to be Staphylococcus epidermidis as causative organism on lid margin. Four eyes of 2 patients showed recurrent superficial punctate keratopathy related to dry eye. We have managed effectively in cases of intractable keratitis and atypical herpetic keratitis revealed S. epidermidis with amniotic membrane transplantation, in diffuse lamellar keratitis with topical steroid, in superficial punctate keratopathy with punctal plug insertion and serum therapy. Because flap formation, damages of stroma and nerve caused by excimer laser keratectomy may mask normal corneal wound healing process and disrupt corneal barrier function, postoperative keratitis may occur. Risk factors in our study were blepharitis and dry eye.<BR> From our experiences, it seems that surgeon who perform the procedure must evaluate carefully all the risk factors and manage properly postoperative vision threatening keratitis(J Korean Ophthalmol Soc 41:2732~2740, 2000).
