성인의 상완골 원위부 과간 분쇄 골절

목적 : 원위 상완골 과간 분쇄 골절에 대해 수술적 치료를 한 환자에서 수술 방식에 따른 치료 결과를 분석하는 데 있다.<BR> 대상 및 방법: 1998년 1월부터 2004년 12월까지 수술적 치료를 했던 원위 상완골 과간골절 환자 22예 중 6개월 이상 추시관찰이 가능했던 15예를 대상으로 하였다. 추시기간은 평균 3년 4개월이었다. 수술 방법은 금속판을 이용한 내고정을 하는 것을 원칙으로 하였다. 수술부 도달법, 금속판의 종류 및 내고정 위치에 따라 Broberg와 Morrey의 평가 지수를 이용하여 각각의 기능적 평가 결과를 비교하였다.<BR> 결과: 기능적 평가는 우수 7예, 양호 6예, 보통 1예, 물량 1예였다. 운동범위는 평균 굴곡구축 7도, 후속굴곡 106도였다. 경주두 도달법을 사용한 경우 평가 지수가 86.6점, Campbell의 후방도달법을 사용한 경우 90.5점이었으며, 재건 금속판과 1/3 반통형 금속판을 이용한 경우 내고정 실패의 1예 (기능지수 57점)를 제외하면 기능지수가 평균 86.3점을 보여 금속판의 종류에 따른 차이는 없었다. 후방과 측방을 고정한 경우 기능 지수가 88.9점이었고, 후방과 후방을 고정한 경우 기능 지수가 86점이었으며, 측방과 측방을 기능지수가 97점을 보여 측방과 측방을 고정한 경우가 가장 우수하였으나 증례수가 적어 통계적 의의는 없었다.<BR> 결론 : 원위 상완골 과간 분쇄 골절의 치료에서 수술부 도달법에 따른 치료 결과의 차이는 없었고, 사용된 금속판의 종류 및 고정방식에 따른 치료 성적의 차이도 없었다. 따라서 골절에 대한 견고한 고정이 가능한 수술적 방법이면서 동시에 시술자에게 익숙하고 환자의 수술 조건에 맞는 적절한 방법을 선택하는 것이 중요하다.

Purpose: To evaluate the therapeutic results of communited intercondylar fractures of the distal humerus that were treated by surgical treatment.<BR> Materials and Methods: From January, 1998 to December, 2004, we reviewed fifteen cases of intercondylar fracture of the distal humerus, which were treated by surgical treatment. The follow up period ranged from six month to 5 years. The functional results were evaluated using Broberg and Morrey`s functional scale according to surgical approach, type of plate and location of plating.<BR> Results: The functional results were as follows; seven excellent, six good, one fair and one poor. The mean range of motion in elbow joint was 7~106°. The mean functional score was 86.6 points through olecranon osteotomy, 90.5 points through Campbell`s posterior approach. The mean functional score was 91.6 points in cases using 2 reconstruction plate, 78 points in cases using 1 reconstruction plate and 1/3 semitubular plate, and 86 points in case using 1 reconstruction plate and lag screws. The mean functional score was 88.9 points in cases by posterior and lateral fixation, 86 points in cases by both posterior fixation and 97 points in case by both lateral fixation.<BR> Conclusion: There are no significant differences in treatment outcome according to surgical approach, different plate and location of plating.
