목적: MR 영상에서 보이는 난소의 양성 성숙낭기형종의 조영증강 고형 성분에 대해 알아보고자 하였다. 대상과 방법: 수술 전 MR 영상을 시행하였고 병리학적으로 양성 난소 성숙낭기형종으로 진단된 32명의 환자 중 조영증강 고형 성분을 보였던 5명의 환자를 대상으로 하였다. 2명의 영상의학과 의사가 후향적으로 MR 영상에서 종양과 종양의 고형 성분에 대해 평가하였다. 결과: 난소 성숙낭기형종으로 진단된 32명의 MR 영상에서 조영증강되는 고형 성분이 있었던 환자는 5명(15.6%)이었다. MR 영상에서 종양의 크기는 평균 9.8 cm이었으며, 종양은 모두 일측성이었다. 종양의 조영증강 고형 성분은 종양의 변연에서 다양한 모습으로 보였다. 종양 주변 장기로의 침범은 보이지 않았다. 결론: MR 영상에서 보이는 난소 성숙낭기형종의 조영증강 고형 성분은 항상 악성 세포전환을 의미하는 것은 아니며 양성 난소 성숙낭기형종에서도 드물지 않게 동반될 수 있다.
Purpose: To evaluate the enhancing solid component within mature ovarian teratomas on pelvic MR examinations. Materials and Methods: Thirty-two women with surgically proven mature cystic teratomas underwent preoperative pelvic MR examinations. Five cases had an enhancing solid component within mature cystic ovarian teratomas on MR images. The MR images were retrospectively analyzed by two radiologists by consensus, focusing on the enhancing portion of tumor and the tumor itself. Results: The study subjects include 5 patients (15.6%) with enhancing solid components within the mature ovarian cystic teratomas. The mean tumor size was 9.8 cm and they were all unilateral. The enhancing solid components of the tumors had a variable appearance and were located in the peripheral region. No cases were found to have a transmural extension or direct invasion of the neighboring pelvic organ. Conclusion: An enhancing solid component associated with mature ovarian teratomas is not uncommon. This finding does not however, necessarily indicate malignant transformation.
