Issues for Method in Estimating Cost of Illness: The Case of Glaucoma

Objective: This study aims to deal with key issues related to the costing method of cost of illness (COI) in the case of glaucoma, mainly focusing on the Korean context. Methods: This study presents the key issues of each cost item with a case for the cost of glaucoma. From a societal perspective, the cost items for calculating COI consist of formal medical cost, informal medical cost, transportation cost, caregiver cost, patient‘s time cost, and productivity loss cost. Formal medical costs of glaucoma were estimated based on the statistical analysis of health insurance claims data. Other cost items (informal medical cost, transportation cost, caregiver cost, patient’s time cost, and productivity loss cost) were calculated through surveys on patients and caregivers. Results: The costs of glaucoma consist of medical costs (formal costs and informal costs), transportation costs, patient-time costs, caregiver costs, and productivity costs due to morbidity. The total cost of glaucoma excluding the productivity costs in 2010 was 252.7 – 308.7 billion KRW. It means that there was a difference of over 20% between results according to the data sources, analytic perspective, and calculating methods. The total cost of glaucoma including the productivity costs is 8,643.5 – 10,350.6 billion KRW. The productivity cost was over 97% of the total cost and it was the cause of huge variation and uncertainty. Conclusion: It is necessary to use the appropriate data sources and calculation methods per cost item when conducting a COI study.

본 연구에서는 녹내장 질병비용의 사례를 통해 질병비용 산출시 주요 쟁점에 대해 다루었다. 사회적 관점에서 비용 항목을 공식적 의료비용, 비공식적 의료비용, 교통비용, 간병비용, 환자시간비용, 생산성 손실비용으로 구분하고, 각 비용 항목별 산출과정에서의 쟁점을 제시하 였다. 먼저의료비용산출시환자정의및자료원에따라동일질병이라도환자수가달라질 수 있다. 또한 비급여 본인부담비용과 비공식적 의료비용에 대한 공식적인 자료원이 없어 추 정 결과에 불확실성이 존재하므로 산출 방법에 대하여 충분히 기술하고 민감도 분석을 수행할 필요가 있다. 교통비용은 적용하는 자료원에 따라 교통비용 단가가 달라지므로 환자 특성 등 을 고려한 자료 선정이 필요하며 간병비용은 비공식적 돌봄의 가치를 추정하는 방법에 따라 결과가 달라지므로 추정 방법을 선정하는데 주의가 필요하다. 질병으로 인한 환자시간비용 산 출 시 시간손실의 범위를 유급 근무시간 손실만으로 볼지 여가 시간의 손실까지를 포함할지는 관점의 차이로 정답은 없으나, 산출 결과에 차이가 크므로 산출 방법과 결과 제시 단계에서 시간손실에 대한 관점도 함께 명시할 필요가 있다. 질병이환 및 조기사망에 따른 생산성손실 수준은 질병의 특성에 좌우되므로 해당 환자를 대상으로 한 별도의 설문조사 등이 필요할 수 있다. 불확실성이크면서일반적으로총비용중중가장큰비중을차지하는생산성손실비 용 결과를 어떻게 제시할지에 대해서는 연구자들간 추가 논의가 필요하다.
