Diagnostic power of the Korean version of Mini-Mental State Examination(MMSE-K) for DSM-III-R dementia was assessed in a large sample of the elderly aged 60 years or more. The sample consisted of the demented elderly(n= 113) and the elderly(n=234) with either mental disorders other than dementia or no mental disorders. At the cutoff point of 24/23,the sensitivity was 92.0% and the specificity was 91.5%. MMSE-K scores overlapped in 23.9% of demented people and 39.3% of the nondemented. With introduction of the arbitrary criteria of ‘ questionable dementia’ (MMSE-K score 21-24) false positive rate was reduced to 1%, false negative rate to 3%. Seventy-four percent of males with Questionable dementia’ score were clinically demented while only 12% of females with the same score were demented.
