The VIP as Patient

In clinical practice, we encountered many difficult situations, especially we have strain of dealing with the entitled person. Current knowledges on the very important person(VIP) as psychiatric patient and VIP syndrome were briefly reviewed respect to the concept, the dynamic formulation( splitting and entitlement), the contributing factors(patient factors, therapist factors, treatment system factors and pathogenic social system factors) and the treatment strategies of the VIP syndrome with discussion of clinical example. VIP syndrome result from dysfunctional intersection of personality and social system factor in which a patient identified as VIP is able to amvey and exaggerated sense of entitlement onto the social field in which the treatment occurs. For treating the VIP psychiatric patient, when attitudes of entitlement and reality-based importance do not interfere with the assumption by the VIP of the patient role, and when the psychiatrist and treatment team are in touch with potential difficulties that attend VIP treatment, then treatment has a good chance for success.
