정신분열병 환자에서 Haloperidol 투여 후 혈청 Prolactin 치의 변동

Prolactin Response to Haloperidol in Schizophrenics

To Investigate the clinical application of prolactin response following antipsychotics challenge in schizophrenics with acute psychotic symptoms, the anthors studied the serial serum prolactin levels before and after intravenous injection of haloperidol 0.5mg in 21 schizophrenics who were drug free for at least two weeks and 20 normal controls. After the baseline were taken at 9 * 00 a.m. on the second hospital day, the blood samples were taken at 45 minutes, 90 minutes and 135 minutes after the injection of haloperidol 0.5mg. The relationship between the prolactin response and such a clinical characteristics of schizophrenia as psychopathology, duration of illness and family history were studied. The baseline prolactin levels in schizophrenics and controls were within normal range but the baseline prolactin levels in schizophrenic group were significantly higher than that of control group. Following haloperidol injection, the prolactin levels at 45,90 and 135 minutes in schizophrenics were significantly higher than those of control group. There was a positive correlation between the negative symptoms and the prolactin levels at 90 minute and 135 minutes but there were no correlation between the prolactin levels and either the positive symptoms of the BPRS total scores. There was no significant difference in prolactin level between the schizophrenics with less than 2 years of illness duration and with more than 2 years of illness duration. Serum prolactin levels at 90 minute and 135 minute in schizophrenics with family history of schizophrenia were singificantly higher than that of without family history. The results suggest that prolactin levels following haloperidol injection are more useful than baseline prolactin levels in understanding the relationship between prolactin response and clinical characteristics in schizophrenics.
