Consultees Concordance with Consultants Psychiatric Diagnoses

In an attempt to characterize consultees representation of psychiatric diagnosis, the medical records of 576 psychiatric consultations done from March 1, 1985 to Feb. 28,1986 were reviewed. Overall referral rate for psychiatric consultation was about 3.48% for inpatients population and higher than that of previous studies. Psychological factors affecting physical condition was the most common psychiatric diagnosis, followed by anxiety disorder, affective disorder, somatoform disorder, successively. Psychiatric diagnosis was recommended to 71% of consulted patients but concordance rate was 50%. Four variables, including consultees, age (40-49), consultanfs status (staff), absence of medical insurance, presence of psychotropic drug recommendation, were significantly related to concordance. But surprisingly concordance was independent of the past psychiatric history, total days hospitalized and timing of consultation. Author observed that consultees were not familiar with the psychiatric diagnosis by DSM-III and concordance decreased strikingly when the patient was diagnosed by the consultant as having a functional mental disorder
