한국어판 SADS 일생형의 타당도에 관한 연구

최근에 정신과 영역의 임상 및 연구분야에 있어 서 진단과정상의 낮은 신뢰도와 타당도에 관한 문 제를 해결하려는 움직임이、활발한 것 같다. 이에 대한 방법상의 문제로 진단기준과 정보수 집 과정의 변인이 중요한 원인으로 대두되고 있으 며，연구 진단기준(Research Diagnostic C rite r ia ; RDC) 이나 DSM-M 의 개발은 진단기준의 변인을 조절하려는 노력이고, SADS일생형(S c h e d u le for A ffective D isorder and S chizophrenia - L ife tim e ; SADS-L)과 미국 NIMH가 개발한 진단적 면담검사(Diagnostic Interview Schedule-Version 3A ; DIS-3A) 는 정보수집 과정상의 변인을 줄이기위 한 구조화된 면담도구이다. 한국어판 DIS- SA에 대한 타당도는 이미 확립 되 어 있으므로, 저자들은 한국어판 DIS-3A 에 대하여 한국어판 SADS-L의 진단상의 일치를 비 교함으로써 SADS 의 타당도를 연구하였다. 연세대학교 의과대학 부속 세브란스병원의 급성 정신과적 입원환자 24명을 대상으로, 두 면담도구 를 사용하여 각각 독립적으로 면담하였고, 각 두 면담의 순서는 임의로 하였으며, 그 간격은 2~ 3일이었다 두 구조화된 면담도구를 이용한 여 러 정신과적 질환에 대한 일치율(Concordance rate) 은 Kappa치로 분석되었으며, 그 결과는 대체로 높은 것으로 나타나 한국어판 SADS-L의 타당도 검증은 만족할만하였다

Two translated structured interviews, the Schedule for Affective Disorders and Schizophrenia- Lifetime (SADS-L) and the National Institute of Mental Health Diagnostic Interview Schedule-version 3A(DIS-3A), were compared for validity of the translated SADS-L. The validity of the DIS-3A Korean version was previously established by Lee et al (1986). Twentyfour new acute inpatients were independently interviewed using the two interview schedules. The order of the interviews was random and they were separated by two to three days. The diagnostic concordance between the two interview schedules for several diagnostic categories was found to be high. There are no establishment of available psychiatric methods to validate psychiatric disorders.
