The dexamethsone suppression te s t (DST) has recently been used not only fo r research but also fo r diagnosis o f depressive illness. There are considerable debates on the accuracy o f cortisol response to lm g and 2mg m edication o f dexamethsone. Thus, the authors trie d to compare system atically the lm g and 2m g procedures in term s o f th e ir accuracy. The study subjects were nineteen patients w ith m ajor depressive disorder who w ere adm itted to the neuropsychiatric wards o f Hanyang U niversity Hospital from September 1, 1985 to February 28, 1986. the results w ere as follows ; 1) No statistical difference in the mean serum cortisol level could be evaluated between the lm g and 2mg procedures. 2) Tw o patients w ere non-suppressors by lm g procedure ; and the same tw o cases (10.5% ) w ere non-suppressors also by 2m g procedure, when the cut-off-point o f 5 ^ g /d l fo r the nonsuppressor was applied. 3) Abnormal cortisol suppression index could be found in 10 patients (52.6% ) from lm g procedure; and 10 patients from 2mg procedure, using sensitiv ity criterion o f 4.0. Consequently, it was suggested th a t the lm g of dexamethasone m edication seemed to be an appropriate procedure for DST.
