The Surgical success of cleft lip & palate treatment depends on the induction of normal growth and function as well as esthetic improvement. Delaire pointed out that conventional cheilorhinoplasty has some difficulties on complete anatomical recinstruction of facial muscles and the failure of postoperative functional recovery have great impact on the unfavorable facial skeletal growth. The functional defecit of nasolabial muscles deteriorates the facial growth and common symptoms of CLP patients-nasal asymmetry, maxillary deficiency, chin deviation toward the cleft side, etc. In these cases, at the time of alveolar bong graft in cleft area, we can usually observe that the great extent of the distortion of facial growth have already processed. Nevertheless, we can find all the facial muscle groups although they are weak and attached to improper anatomical position. If we reconstruct the functional orientation of nasolabial muscles through the functional cheilorhinoplasty at the time of alveolar bone graft, deterioration of unfavorable facial growth can be avoided maximally as possible. In addition, lip incision and retraction guarantee the wide operation field at the time of bone graft and enough amount of cancellous bone can be filled into cleft defect area when alveolar bone graft is performed with functional cheilorhinoplasty simultaneously. so, we present the cases of functional cheilorhinoplasty with alveolar bone graft in CLP children in these purposes
