This study was designed to support the conception of delusional unipolar depression as a distinct subtype of depressive illness. The clinical course, symptoms and treatment response of 13 delusional and 74 nondelusional unipolar depressed patients were compared in a retrospective analysis. The results were as follow s: 1) Age at onset of the delusional patients was older than that of the nondelusional patients (ttest,p < . 05). 2) The delusional patients had more self-reproach, insomnia and were more ruminative, agitated and referential than the nondelusional patients (p < . 001,. P < . 05, p < . 05, p < . 01, p < . 001). 3) The delusional patients had poor responses to, tricyclic antidepressant therapy or tricyclic-antipsyychotic combination therapy compared w ith nondelusional patients (p < . 001, p < . 05).
