We experienced a case of negative schizophrenia responding to combined therapy of typical antipsychotics and mianserin. Typical antipsychotics are believed to exert their antipsychotic effects by blocking dopamine receptors or by depleting dopamine. Mianserin is a tetracyclic antidepressant, and its pharmacological profile is characterized by the potent antagonism of postsynaptic 5-HT2 receptors. Despite an increase of reports on the treatment of negative symptoms by medication, there are at present no convincing data which medication is effective in the treatment of negative schizophrenia. We suggested that mianserin should be useful in the treatment of the negative symptoms of schizophrenia and its mechanism should be dynamic interaction between serotonin system and dopamine system.
