만성 정신분열병 남자환자의 성기능에 대한 연구

A Study of Sexual Function in Male Patients with Chronic Schizophrenia

Objective The purpose of this study was to explore the effects of illness itself and antipsychotics u pon sexual functioning in male patients with chronic schizophrenia. Methods : The 87 patients were assessed by Sexual Anxiety Inventory(SAI), Index of Sexual Satisfaction(ISS), Brief Psychiatric Rating Scale(BPRS), Sexual Functioning Questionnaire, Abnormal Involuntary Movem ent Scale(AIMS), serum prolactin level and clinical data. Results 1) Sexual anxiety of schizophrenic group was higher than that of the control group and not different according to marital status. The degree of sexual satisfaction was lower in schizophrenic group, especially in the single. As usual sexual functioning over the past 2 years and sexual functioning over the past 2 weeks, average frequency of sexual thoughts, erections and masturbations in the schizophrenic group were lower than those of the control group. 2) Total score of SAI and ISS of schizophenic group had significant positive correlation and correlation between usual functioning over the past 2 years and sexual function over the past 2 weeks was high also, bu t sex-related factors such as sexual anxiety, sexual satisfaction and sexual functions had no significant correlation with various clinical variables such as duration of illness, num ber of admission, etc. Usual sexual functioning over the past 2 years and sexual functioning over the past 2 weeks were negatively correlted with number of admission and total score of BPRS. Patients and Interviewers assessment score for sexual dysfunction related with present medication were also negatively correlated with total score of AIMS respectively. 3) As sexual anxiety, sexual satisfaction and changes in sexual function except sexual functioning over the past 2 weeks were not different between two groups which were divided by potency of antipsychotics. 4) In present sexual impairment related with antipsychotics, patient’s assessment score correlated negatively with change in sexual thoughts and intervewers assessment score negatively with change in the frequency of erections. Both assessment scores were correlated with total score of AIMS , but not significantly correlated with serum prolactin level and other clinical variables. Conclusion : These results may suggest that male chronic schizophrenia have more sexual problems such as higher sexual anxiety and lower sexual satisfaction to compare with the control group, bu t various clinical variables are not related to these dysfunctions. Usual sexual functioning over the past 2 years and sexual functioning over the past 2 weeks are related with psychiatric symptoms, especially thought disturbance and present sexual impairments are related with drug induced movement disorder. In effects of medication on present sexual impairment, patients subjective assessment is ‘decreased in sexual thoughts’ and interviewers objective as sessment is decreased in erections • There is not significant correlation between present sexual impairment with serum prolactin level which is pharmacologic indicator of dopamine blockade.
