사회공포증에 관한 10년간의 임상연구

A Clinincal Study of Social Phobia for 10 Years

The authors examined the clinical manifestations of 838 patients who had been diagnosed social phobia at Koryo General Hospital from May 1982 to Apr. 1992. We discussed the patients’ demographics, clinical characteristics and results of group therapy. Some of the important findings are as follows : 1. Demographic characteristics. 1) Social phobic patients comprised 4.5% of the total numbers of psychiatric outpatients. 2) Male patients(521 cases) outnumbered females(317 cases) with ratio of 1.6 : 1. 3) Majority, 454 cases(54.2%) were classified as simple type, 270 cases(32.2%) as type of ideas of harming others and 114 cases(13.6%) as borderline type. 4) The mean age of initial hospital visit was 26.4 and the mean age of symptom onset was 19.5. 2. Clinical characteristics. 1) Four most common symptoms, according to their frequency, were tensing up(22.2%), fear of one’s own gaze(l5.6%), blushing(l4.3%) and trembling of hands(l2.1%). 2) As to the onset of symptoms, those who developed with gradual onset were most frequent (52.7%). 3) Most of the patients complained of having hard time in their social life owing to their phobic symptoms and 14.1% of patients had history of suicide attempt 3. Results of group therapy. 1) The fear of self emitting order showed good treatment response than trembling of hands, and trembling of voice than trembling of hands and facial expression. 2) Group therapy was effective in both types, simple and ideas of harming others. However, poor treatment outcome was noted in the borderline type. 3) Cognitive reconstruction through therapist’s lecture was most effective in simple type, and paradoxical intention was more effective in the type of ideas of harming others than borderline type.
