한국에 있어서 Mini-Mental State Examination (MMSE) 의 표준화 연구

Standardization of the Mini-Mental State Examination(MMSE) in Korea

The authors translated MMSE written by Folstein et al. into korean and performed standardization for the purpose of the application in Korea. The subjects were 53 organic mental disorder patients, 45 schizophrenia patients, 43 depression patients and 27 mania patients. For interrater reliability, two psychiatrists applied Korean version of MMSE to 53 organic mental disorder patients individually at the same time, and calculated Kappa of each item of MMSE. To find out the difference among organic mental disorder, schizophrenia, depression and mania, t-test was done based on the score of the total and each item between organic mental disorder and each of the other group. Discriminant function analysis was done to differentiate organic mental disorder from each of the other group. Also, cut-off points and its diagnostic validities in organic mental disorder and functional mental disorder(schizophrenia, depression and mania) was calculated. The results were as follws: 1) As for the interrater reliability of Korean version of MMSE, the Kappa values of naming, expressive speech and registration were 0.65, 0.65 and 0.64, respectively, which suggest good reliability. And those of remaining 16 items of MMSE were ranging from 0.80 to 1.00, which suggest excellent reliability. Also Kappa value of total items was 0.86,which suggest excellent reliability. 2) There were statistically significant differences in the total scores of MMSE between organic mental disorder and schizophrenia, organic mental disorder and depression, organic mental disorder and mania patients(P<0.001). 3) Accuracy rates of discrimination between the group of organic mental disorder and each of the other group were as follows: It differentiated organic mental disorder from schizophrenia with 92.86% accuracy, from depression with 93.75*% accuracy, and from mania with 93.75^ accuracy. 4) We conclud that cut-off value of organic mental disorder and functional mental disorder (schizophrenia, depression and mania) in this study was 24/23 scores(sensitivity 88.6%, specificity 92.1%).
