Olanzapine을 투여 받은 정신분열병 환자의 체중, 체질량지수, 렙틴, 인슐린, 공복시 혈당의 변화

Body Weight, Body Mass Index, Plasma Leptin, Insulin and Fasting Glucose Level in Schizophrenic Patients Receiving Olanzapine

연구목적： 본 연구는 olanzapine의 투여와 관계된 체중증가의 가능한 기전을 규명하기 위하여 시행하였다. 방 법： 정신분열병으로 진단 받고 순천향대학교 천안병원 정신과에 입원 중인 13명의 환자를 대상으로 체중, 체질량 지수(body mass index：BMI), 렙틴(leptin), 인슐린(insulin)과 공복시 혈당(fasting glucose)을 olanzapine 투여전과 투여 후 1주, 2주, 4주, 6주에 측정하였다. 결 과： Olanzapine을 투여 받은 환자는 체중, 체질량 지수, 렙틴, 인슐린, 공복시 혈당이 모두 투여전과 비교하여 의 미있게 증가하였다. 인슐린과 렙틴은 olanzapine 투여 전에 비해 치료 6주째에 유의하게 증가하였다. 체중과 체질량 지수는 투여전과 비교하여 각각 1주, 4주, 6주째에서 유의하게 증가하였다. 공복시 혈당은 투여 전에 비해 4주와 6주째에서 의미있게 증가하였다. 상관관계분석에서 혈장 렙틴과 인슐린, 체중과 인슐린, 체질량 지 수와 인슐린, 공복시 혈당과 인슐린이 각각 유의한 상관관계를 나타내었다. 렙틴과 체질량 지수는 상관성의 경 향을 나타내었으나 렙틴과 체중은 상관관계를 보이지 않았다. 결 론： Olanzapine 치료는 체중증가와 연관이 되고 혈장 렙틴, 인슐린, 공복시 혈당을 증가시키는 것과 관련이 있었 다. Olanzapine에 의해 유발된 인슐린 분비 증가와 렙틴의 증가가 체중증가의 원인적 기전일 수 있을 것이다.

Objective：The aim this study was to investigate possible mechanisms behind olanzapine-associated weight gain. Method：Thirteen psychiatric inpatients(all meeting DSM-Ⅳ criteria for schizophrenia or schizophreniform disorder) receiving olanzapine in Soonchunhyang university Chun-an hospital were studied. Body weight, plasma leptin, insulin and fasting glucose levels were measured five-times over 6 weeks. Body mass index(BMI) was calculated. Result：At the end of sixth week, body weight, BMI, plasma leptin, insulin and fasting glucose levels were significantly increased from baseline. Both insulin and leptin plasma levels were significantly increased from baseline at the end of the sixth week of treatment. Both body weight and BMI were significantly increased from baseline at the end of the first week, the fourth week and sixth week, respectively. Fasting glucose was significantly increased from baseline at the end of the fourth week and sixth week, respectively. Between baseline and sixth week, the body weight, BMI, plasma leptin and fasting glucose levels were significantly correlated to insulin level. There was a tendency toward a correlation between BMI and leptin level, whereas no correlation was found between body weight and leptin. Conclusion：Olanzapine treatment was associated with weight gain and elevated level of leptin, insulin and fasting glucose. Elevated insulin levels and hyperleptinemia may be mechanisms behind olanzapine-induced weight gain.
