연구목적： 난치성 정신분열병 환자에 사용되는 클로자핀의 용량이 인종적인 차이를 보일 수 있다는 이전 연구 결과 들을 바탕으로 한국인 성인 정신분열병 환자의 클로자핀 사용량을 치료기간 별로 나누어 조사해보고 사용량 과 클로자핀 및 그 대사물의 혈중 농도를 조사하여 한국 성인 정신분열병 환자의 적정 클로자핀 용량을 알아 보고자 한다. 방 법： 클로자핀으로 치료한 성인 정신분열병 환자 중 사용기간이 1년 이상 되는 환자들을 대상으로 사용 기간 별(2주, 4주, 8주, 12주, 24주, 52주)로 나누어 평균 사용량을 조사하고 이전에 보고된 외국인의 용량과 비교 를 했다. 흡연 유무와 성별 차이를 조사했다. 클로자핀 및 그 대사물의 혈중 농도는 HPLC를 사용하여 측정 했다. 결 과： 2, 4, 8, 12, 24, 52주째 측정한 사용기간 별 평균 클로자핀 양은 각각 222.5(±92.2), 330.0(±139.2), 344.8(±80.6), 327.5(±93.4), 288.3(±118.5), 239.3(±112.3)mg/일 이었다. 성별 비교에서 여자 환자는 클로자핀 사용 8주 후 남자에 비해 유의하게 평균 사용량이 낮았다. 흡연자의 클로자핀 사용량이 비흡연자에 비 해 다소 높았으나 통계적인 차이는 보이지 않았다. 클로자핀 및 그 대사물의 혈중 농도는 평균 사용량과 유의한 상관관계를 보였고(p＝.000), 동일한 클로자핀 용량에서 혈중농도는 성별, 흡연 유무와 차이가 없었다. 결 론： 한국인 성인 정신분열병 환자의 클로자핀 사용 평균 용량은 이전에 보고된 서양인(Caucassian)보다는 낮 고, 치료적 유효 혈중농도 350∼400ng/ml을 유지하기 위한 클로자핀 양은 300∼400mg/일 이었다.
Objectives：The present study aimed to determine the optimal clozapine dosage in Korean adult schizophrenic patients, and to measure the blood levels of clozapine and its major metabolites. Methods：Thirty schizophrenic patients, who have undertaken clozapine over 1 year, were recruited in this study. We investigated the clozapine dose at different times(2, 4, 8, 12, 24, 52 weeks) during the course of treatment with clozapine and we measured the blood level of clozapine and its major metabolites, N-desmethylclozapine and clozapine-N-oxide by using high performance liquid chromatography( HPLC). Results：The average dosages of clozapine at 2, 4, 8, 12, 24, 52 weeks were 22.5(±92.2), 330.0 (±139.2), 344.8(±80.6), 327.5(±93.4), 288.3(±118.5), 239.3(±112.3)mg/day respec-tively. The average clozapine dose of male schizophrenics was significantly higher than that of female schizophrenics after 8 weeks during the course of treatment. But there were no significant differences in blood level of clozapine and its major metabolites between genders in same dosage of clozapine. The average dose of clozapine in smokers was slightly higher than non-smokers, but there was no statistical significance. The blood levels of clozapine and its major metabolites was significantly correlated with the total dose of clozapine. Conclusion：The average clozapine dose of Korean adult schizophrenics was lower than that of reported Caucasian data, and to reach the known therapeutic clozapine blood level above 350–400 ng/ml, we need 300–400mg/day of clozapine
