신경성 대식증이 Russell에 의해 신경성 식욕부진증과는 별개의 독립된 문제로 기술된 이후 이병은 특히 서구 사회를 중심으로 지난 20여년에 걸쳐 발생율이 급작스럽게 증가되는 문제로 알려져 왔다. 그러나 최근 에 들어 여러 연구들이 경제적 발전에 동반된 서국사회 문화의 빠른 도입이 한국을 비롯한 개발도상국에서 신경성 대식증을 비롯한 식사 장애의 발병율이 증가되고 있다고 보고하고 있다. 신경성 대식증 환자들의 치료는 외래치료를 원칙으로 한다. 병발증이 동반되지 않은 신경성 대식증 환자의 경우 입원치료는 거의 필요하지 않고 영양 상담 및 영양 회복, 개인 및 집단 인지 행동 치료, 행동 치료, 정 신 분석 치료, 정신 역동적 접근, 가족 치료 및 약물 치료 등으로 임상적인 호전을 얻을 수 있다. 그러나 임상 실제에 있어서는 외래 치료를 중심으로 한 인지 행동 치료와 대인 관계 치료가 주로 이루어지고 있고 여기에 폭식의 조절이나 병발 장애를 치료하기 위하여 항우울제를 주로 하는 약물 치료가 부가되는 것이 흔하게 이 용되는 방법이다. 본 논고의 목적은 신경성 대식증 환자의 치료에 핵심이 될 수 있는 임상 지침을 체계적으로 정리하고 본 병원 식사 장애 클리닉에서의 신경성 대식증 환자의 치료 경험을 소개하는데 있다.

Since Russell described bulimia nervosa as an independent and distinctive eating problem, it has been regarded as one of the rapidly increasing problems during the past two decades in Western society. However, in the past few years, the number of reports concerning eating disorders have increased in developing countries including Korea with the rapid introduction of the Western culture accompanied by economic development. Most individuals suffering from bulimia nervosa can be treated as outpatients. Patients with uncomplicated bulimia nervosa rarely require hospitalization and may achieve clinical improvement with nutritional management or counseling, individual or group cognitive-behavioral therapy(CBT), behavioral techniques, and interpersonally oriented or psychodynamically oriented psychotherapies. However, in clinical practice, CBT with nutritional counseling or inter-personal psychotherapy is a usual method chosen for the treatment of such patients in combination with pharmacotherapy. This review aimed at giving a compact and systemized account of the clinical guidelines that we consider essential in the management of patients with bulimia nervosa.
