화상 환자는 화상후 경과 시간에 따라 차례로 생리적 위급의 시기, 심리적 위급의 시기, 사회적 위급의 시 기를 경험하게 된다. 본 연구에서는 88명의 환자들을 위의 세 단계로 구분하여 각 단계별로 불안, 우울, SCL-90-R을 이용한 다양한 측면에서의 심리 반응, 병동내 활동상황, 화상후 신체상의 변화를 살펴 본 결 과 다음과 같은 결론을 얻었다. 1) 화상 후 불안은 시일 경과에 따른 단계 간에 차이를 보이지 않았다. 우울은 화상후 퇴원전까지 시일이 지나감에 따라 단계적인 증가를 보였으며, 가족내 갈등, 저학력 등이 변이 요소로 작용하였다. 2) SCL-90-R로 조사한 결과에 의하면 stage Ⅰ과 Ⅲ간에 불안, 우울, 신체화, 공포불안 척도에서 유의 한 차이를 보였다. 3) 간호사가 관찰한 환자의 우울 정도는 환자가 실제로 느끼는 우울, 불안과 경한 정도의 상관 관계를 보였고, 간호사가 관찰한 치료에 대한 협조도는 환자가 느끼는 우울의 경중과 경한 정도의 상관 관계를 보였다. 4) 화상후 신체상에 대한 불안이 매우 높아졌는데, 시기별로는 유의한 차이가 없었다. 신체상의 불안에 영 향을 미치는 변이 요소는 화상의 면적, 재원 기간, 피부가 검게 변한 합병증 등이었다. 5) 화상 환자들은 이같이 정신 심리적인 문제를 갖게 되는데, 이를 위해서 화상 치료시 정신 심리적인 치 료나 지지가 가능하도록 정신 의학적인 접근이 절실함을 보여주고있다.
Burn patients basically go through the sequential stages of physiological, psychological and social emergency after the burn injury. In the present study, 88 burn patients were classified into three groups according to the stages as mentioned above. Then, we observed the states of anxiety, depression, inward activities, various psychological responses measured by SCL-90-R, and body image changes in each stage. The study results were as follows： 1) We found that depressive symptoms increased as the stage advanced. They were correlated with family conflicts and low level of education. The state of anxiety was not correlated with the staging. 2) There were differences between stages I and Ⅲ in ANX, DEP, SOM, PHOB axes of SCL-90-R. 3) The mood of burn patients and the patients’ cooperation to the treatment observed by the nurse was partially correlated with depression and anxiety, while the patients’ relation to others and interpersonal relationship were not. 4) Variables such as the size of burned area, duration of hospitalization and darkening skin affected the patients’ body image. 5) We found that burn patients have psychological difficulties. Therefore, the treatment of burn injury should be done in parallel with the psychological treatment by professionals.
