Clinical Predictors of Clozapine Response in Schizophrenics

정신분열병 환자에서 clozapine 치료 전이나 치료 초기에 약물의 치료 반응을 예측할 수 있다면, clozapine 투여 대상 환자를 보다 선택적으로 선정할 수 있을 것이므로, clozapine에 의한 무과립구증의 발 생 빈도를 감소시킬 수 있을 것이다. 본 연구는 이러한 임상적 필요성에 따라, clozapine 치료 전이나 치료 초기에 약물의 반응을 예측할 수 있는 특징적인 임상 징후를 찾고자 고안되었다. 연구 결과를 요약하면 다음과 같다. 첫째, clozapine 투여 6주후 CGI 점수가 약물 투여 전 기초 평가시의 CGI 점수에 비해 1점이상 감소하였 을 경우에 clozapine 반응자로 분류한 결과, 반응군은 24명이었고 비반응군은 17명이었다. 이러한 결정은 통계적으로 타당하였다. 둘째, 약물 투여 전 기초 평가시의 BPRS 점수를 양군간에 비교한 결과, 약물 반응군에서 BPRS 총점수뿐 만 아니라 BPRS 세부 항목 가운데 불안, 적개심, 환각, 괴이한 행동, 자기관리소홀, 흥분 항목점수가 비반응 군에 비해 유의하게 높았다. 세째, 반응군에서는 약물 투여 2주후 BPRS 세부 항목 가운데 불안과 기이한 사고 내용 항목 점수가 약물 투여 전 기초 평가시보다 유의하게 감소하였다. 결론적으로, 본 연구를 통해 정신분열병의 전반적인 정신 병리의 심한 정도뿐만 아니라 특히, 불안, 적개심, 환각, 기괴한 행동, 자기관리소홀, 그리고 흥분 증상들이 clozapine의 치료 효과를 사전에 가늠해 볼 수 있는 유효한 예측 인자가 될 수 있을 것으로 생각되었다.

The identification of clinical predictors of clozapine response in schizophrenics might be helpful to make a selection of the patients for clozapine treatment, and could also be useful to diminish the incidence of agranulocytosis, an unpredictable and potentially lethal side effect of clozapine. This study was designed to identify the characteristic symptoms which can predict a good response to clozapine in schizophrenics before or in early stage of clozapine treatmnet. The results were summarized as follows. First, while twenty four patients who had a reduction of 1 point or greater in CGI scores from baseline to sixweek clozapine treatment were defined as clozapine responders, seventeen patients without such a change were defined as clozapine nonresponders. The classification between the two groups was assessed to be valid statistically. Second, both baseline BPRS total scores and each score of 6 BPRS items such as anxiety, hostility, hallucinations, bizarre behavior, self-neglect, and excitement were significantly higher in the clozapine responders than in the clozapine nonresponders. Third, in the responders after the second week of clozapine treatment, there was a significant reduction from baseline on the anxiety and unusual thought content item scores. In conclusion, several symptomatic factors could be regarded as clinical predictors of clozapine response in schizophrenics, including anxiety, hostility, hallucinations, bizarre behavior, self-neglect, and excitement as well as the severity of psychoticism
