The use of oral implants for prosthetic devices has increased the therapeutic possibilities in many aspects. However, inappropriate anatomic conditions hinder implants to be placed on the alveolar bone. Success of the implant treatment depends on the presence of sufficient quantities of bone and favorable intermaxillary relationship. Even when sufficient bone is present, the shape of the alveolar process, the position of the mandibular nerve, and the existence of the maxillary and nasal sinus can still be a complicating factor in implant surgery. In the case of deficient ridges, onlay graft can be a choice. In horizontal narrow alveolar ridge, ridge-splitting technique also can be used. To compete with other handicapped problems, it is advisable to ecreate a better anatomic environment for the implant using GBR, sinus lift and nerve repositioning techniques. The purpose of this article presents an overview of several applying techniques and rationale for our experienced cases.
