The restoration of function and esthetics of the atrophic mandible has been therapeutic challenge for surgeon and restorative dentist. Endosteal implants are indicated for dental rehabilitation for the atrophic mandible, but there have possible complications such as mandible fracture, implant loss, fracture of implant superstructure, marginal bone loss, and paresthesia of the mental nerve, etc. Bone graft combined with the endosseous implant in severely atrophic mandible decrease these complication and so more functional and esthetic rehabilitation can be possible. Extraoral approach minimizes the possibility of oral contamination, and subsequent infection and grafted bone loss. We report a case of reconstruction of the severely atrophic edentulous mandible by means of autogenous bone graft and osseointegrated implants with submental incision simultaneously, and successful clinical experience was obtained, and we report with literatures review.
