자연치-implant 지지 konus denture를 이용한 교합회복

Statement of problem Since the implant treatment was introduced in 1970s, the rehabilitation of difficult cases has been possible, however not all patients can have implants for economical, anatomical limitation and residual ridge conditions. We should be able to provide solution to these patients with minimum number of implant fixtures. Purpose For achieving satisfactory mastication of patients with severely missing dentition, we can apply the concept of rigid support of the konus denture supported by the natural tooth and implant abutment. Material and method A young female patient with floating teeth in upper and lower arch without any specific general disease was treated with upper and lower konus denture. Upper denture is supported only by natural teeth, right 2nd molar, left bicuspid and left 2nd molar. In the lower arch there were few sound teeth. So in left 2nd molar and right bicuspid area, the strategic position for bilateral stabilization of konus denture, implant fixtures are placed. So as not to increase the lateral force on implants, magnetic attachment was used. Magnetic keeper was also placed on the left bicuspid root rest. After placing inner crown on the right 3rd molar, konus denture was made. Results After 12months the periotest value was from -4 to +2,this value is clinically acceptable. The patient was satisfied with the retention, ability of chewing, facial esthetics and hygienic maintenance. Occlusal force was measured by occlusal force-meter GM10, (Nagano Keiki Co.LTD.). Maximum occlusal force was 143N, ranged from 97-143N on the bilateral molar site. The value is within the range of the normal occlusal force and there is no dislodging of denture when biting this instrument, so the retention of this denture was sufficient. Conclusion Konus denture supported by natural tooth and implant abutment provide satisfactory retention, support, facial esthetics, ability of mastication and easy hygienic maintenance.
