임프란트를 이용한 BAR OVERDENTURE 장착환자에서 VESTIBULOPLAST丫를 이용한 부착치은 증대술

Sometimes we use implant supported bar overdenture at severely resorbed alveolar ridge, unfavorable jaw relations, and cosmetic and phonetic problems are present, because it has much benefit. But there are aiso some complications: such as stomatitis, ulcer decubitus, hyperplasia, attached gingival recession, etc. Attached gingiva acts as a buffer to movable alveolar mucosa or muscle attachment, and prevents a direct influence of pulling motion on tissue around an abutment. If an abutment is placed in the area through alveolar mucosa, a pocket between the abutment and mucosa may be an area for plaque accmulation. A chronic inflammation and muscle movement pulling could create traumatic area. The traumatic forces contribute to the inflammatory proccess and subsequent marginal bone loss. In general if attached gingiva amount is below 5mm, we do operation to ensure ample amount of attached gingiva before implantation. This time we experienced a patient wearing implant supported bar type overdenture with severe attached gingival recession and gingival hyperplasia, and we report this case to emphasize the importance of the attached gingiva easily can be neglected.
