임부의 사회적지지, 배우자폭력과 건강관련 삶의 질의 관계에서 산전우울의 매개효과

The Mediating Effect of Antenatal Depression Between Social Support, Intimate Partner Violence and Health-Related Quality of Life among Pregnant Women

본 연구는 임부의 건강 관련 삶의 질에 영향을 미치는 요인에 대한 선행연구를 근거로 임부의 건강관련 삶의 질에 관한 가설적 모형을 구축하고, 적합성 정도와 연구가설을 검정하는 구조모형 연구이다. 2016년 9월 2일에서 17일까지 D, P시 소재 산부인과 병원 3곳에 내원한 임부 238명을 대상으로 구조화된 설문지 조사를 실시하였다. 모형의 적합도는 ｘ2 =140.15(df=71, p<.001), GFI=.956, NFI=.915, NNFI=.943, RMR=.415, SRMR=.051, RMSEA=.064이었다. 모형 검증 결과 사회적지지는 배우자폭력, 산전우울, 임부의 건강관련 삶의 질에 직접적인 영향이 있었고, 또한 산전우울을 매개로 하여 건강관련 삶의 질에 영향을 주었다. 배우자폭력은 산전우울을 매개하여 임부의 건강관련 삶에 간접적으로 영향을 미쳤으나, 직접적인 영향을 나타내지 않았다. 본 연구결과를 통하여 임부의 건강 관련 삶의 질을 증진시키기 위해 산전관리 시 사회적지지, 배우자폭력, 산전우울을 사정하고, 특히 임부의 산전 우울의 예방 및 관리를 하여야 할 필요성을 제기한다.

This study was done to develop a predicting model for health-related quality of life among pregnant women. It confirmed through the structural equation whether the field in the antenatal depression did the mediating role in the health related quality of life of the pregnant woman. Constructed questionnaires were completed by 238 pregnant women attending antenatal clinics in D and P cities from September 2th to September 17th in 2016. Fit indices confirmed that the hypothesized model was a good representation of the observed data ｘ2=140.15 (ｘ2/df=71, p<.001), GFI=.921, CFI=.956, NFI=.915, NNFI=.943, RMR=.415, SRMR=.051, RMSEA=.064. Social support had statistically significant effects on intimate partner violence and health-related quality of life among pregnant women. Antenatal depression had significant mediating effects on health-related quality of life among pregnant women from social support and intimate partner violence. The results of this study suggest that social support, intimate partner violence and antenatal depression in antenatal care should be assessed to improve the health related quality of life of pregnant women, and the prevention and management of antenatal depression should be promoted.
