복합외상(C-PTSD)을 경험한 청소년의 모래놀이치료 사례연구

A Case Study on the Sandplay Therapy for an Adolescent who Underwent Posttraumatic Stress Disorder

본 사례는 고등학교 2학년 청소년의 사례로 초등학교 3학년시기에 모(정신지체3급)를 대신하여 애착관계를 형성한 주 양육자인 조모의 음독사를 직접 목격하였고 외상후스트레스장애 (PTSD)진단에 부합되는 증상을 보였음에도 적절히 치료 받지 못한 상태로 성장한 청소년 사례이다. 본 사례의 연구방법은 내담자의 모래놀이 치료과정에서 구성한 모래놀이 작품과 작품에 대한 설명 내담자의 언어표현, 치료자의 느낌, 수퍼비전 내용 등을 토대로 사례연구를 시행하였다. 본 사례의 특징은 외부적 지원환경, 즉 학교복지지원에 의해 12회기 계약 회기를 정하고 시행된 사례로 연구의 목적은 모래놀이치료가 복합외상 및 외상후스트레스 증상을 보인 청소년에 있어 모래놀이의 효과성 및 12회기 계약싱투} 진행에 대한 치료적 접근성에 관해 살펴보고자 한 연구이다. 본 연구의 결론은 아동기 주 애착대상의 상실과 같은 외상사건에 노출된 경우 행동의 변화와 함께 심리 정서적 문제를 동반하는 증상을 보이며, 아동의 경우 사건 초기에 외상사건과 관련한 적절한 치료적 접근이 필요하고 이에 모래놀이치료가 효과성이 있는 것으로 보여 진다.

This case involves an adolescent in the second year of high school. In his third year of elementary school, he witnessed his grandmother s death from poison drinking, who formed attachment relationship with him instead of his mother (third degree mental retardation). Thereafter, the child, who was active in peer relationship and external activities, became inactive and intimidated in interpersonal relationship and exhibited fear and defensive attitudes in relationship with meaningful subjects. His condition corresponded with posttraumatic stress disorder (PTSD). Psychological difficulties (anxiety, depression) were not treated in an appropriate period, problematic behaviors appeared, cognitive stimulation deficiency and lack of appropriate educational support affected his development, with immature development. Therefore, it is a case where sandplay therapy helped ego development and emotional development intimidated by psychological difficulties caused by traumatic damage and formed an independent ego identity that should be obtained during his adolescence. This case study was performed based on sandplay works created by the client in the process of sandplay therapy, explanation about the works, the client s linguistic expressions, the therapist s feelings, and content on the therapist s supervision. The theoretical background of this case was an analytic psychological theory that if an appropriate environment is given, psyche heals itself and has an autonomous disposition to grow completely uttered by Jung (1960). This study helped the child s ego develop in sandplay therapy through support and accommodation centered on mother-son unity by Dora Kalff.
