We report on a case of acute cholangitis associated with scrub typhus. A 66-year-old man was admitted because of right upper quadrant abdominal pain, fever, and general myalgia. Whole abdominal ultrasonography and computed tomography revealed diffuse dilatation of the common bile duct (CBD) with multiple stones. Acute cholangitis was strongly suspected and proper antibiotic treatment of scrub typhus was started later. This case demonstrates the importance of consideration of tsutsugamushi disease based on the patient s history of exposure, and clinical features in endemic areas.
