수술 후 운동요법이 척추수술환자의 자기효능감과 일상생활기능에 미치는 효과

The Effects of Exercise after Surgery on Self-Efficacy and Activities of Daily Living of Spine Surgery Patients

Most of spine surgery patients meet with good results through operative cures but some of them show symptoms of low back pain, sciatica, dysesthesia after surgery. Sometimes those symptoms are recurrent after a certain period of time. It disturbs patients in daily living or work living and also causes social and economical problems and even if it passes into a chronic state, it also causes psychological problems like a losing desire, the lack of self-confidence, depression or anxiety. Therefore, patients need rehabilitation therapy through exercise to mitigate those problems. We can decrease the time for recovery with increasing of muscle strength and limberness and decreasing of pain and disturbance of daily living through exercises after spinal surgery. This study has been attempted to identify the effect of exercise after surgery on self-efficacy and activities of daily living of spine surgery patients and to provide basic data for practice of exercise as a nursing intervention for spine surgery patients. The present study was a nonequivalent control group pre-test and post-test design in quasi-experimental research. The data was collected from December 11, 2002 until April 10, 2003 and the subjects were spine surgery patients who had been hospitalized in neurosurgery of Y university hospital in Seoul, 13 assigned for the experimental and 12 for the control group. Exercise of this study reformed to be adapted to spine surgery patients on the basis of William s flexion exercise and exercise for HLD surgery patients developed by Kim, Hyun Ju(1995) and was practiced for 5 weeks, two times a day. The effects of exercise was measured with self-efficacy and activities of daily living and the data was analysed with x2-test and Mann-Whitney U test using the SPSS program. The results were as follows 1. Hypothesis 1, “The score of self-efficacy of experimental group which took exercise after surgery will be higher than the control group”, was rejected (z=-1.007, p=.157). 2. Hypothesis 2, “The score of daily living activities of experimental group which took exercise after surgery will be higher than the control group“, was supported (z=-1.732, p=.042). Thus exercise after surgery was not effective on increasing self-efficacy of spine surgery patients, but was effective on improvement of daily living activities. So, we need to research repeatedly the effect of exercise on spine surgery patients and nurses should make every effort to develop exercise and train it for them.
