요양시설 거주노인의 구강위생상태 향상을 위한 간호인력 실무훈련 교육프로그램 적용 및 효과평가

Application and effectiveness of practical training program for nursing caregiver for improving oral hygiene status of the elderly in nursing-home

본 연구는 노인요양시설에서 간호제공자들이 올바른 구강 간호술 교육을 받은 후 실무에 적용했던 경험을 확인하기 위한 질적 연구이다. 연구대상자는 구강간호교육을 이수한 노인요양시설 간호제공자 22명이었다. 교육받은 구강간호내용을 적용한 경험이 있는 간호제공자들을 대상으로 비구조적이고 개방적인 질문을 통해 ‘구강간호 관심가지기’,‘구강관리 성공경험’,‘구강간호 적용의 두려움’,‘구강관리 지지체계 부족 인지’,‘의치관리의 어려움 인지’,‘ 다양한 경우에 대한 정보요구’,‘ 대처방법에 대한 경험나누기’의 7개 범주가 도출되었다. 노인요양시설 간호제공자들은 구강 간호에 대한 교육받고 실무에 적용하고자 하였으나 주변의 노인 구강간호에 대한 무관심이나 미흡한 구강관리 지지체계 등으로 인해 어려움을 겪었으며 개인적으로 직접 배운 내용을 적용하는데 두려움을 느꼈으나 어려운 대상자에 대한 성공경험이 있는 경우 자신감을 가지게 되었다. 교육프로그램을 통해 배운 구강간호를 제대로 적용할 수 있도록 시설의 지지체계를 보완하고 성공 경험을 촉진할 수 있는 현장 멘토가 필요할 것으로 여겨진다.

This study is a qualitative study to describe the experiences of nursing caregivers applying the oral hygiene care in elderly nursing home received oral hygiene care education. The subjects were 22 elderly nursing home caregivers who received oral hygiene care education. Focus group interviews were conducted on non-structural and open-ended questionnaires for caregivers who had applied oral hygiene care as educated. All interviews were analyzed by content analysis as qualitative research method. Seven categories were conceptualized: indifference to oral hygiene care for elderly , poor supportive system of oral hygiene care in facilities fear of application of oral hygiene care and‘successful experience of oral hygiene care . The care providers tried to apply the educated oral hygiene care practice to the elderly but they had difficulties in application due to indifference to the oral care of the elderly or lack of oral hygiene care support system and felt the fear of applying the educated contents. But they had confident in case of having a successful experience with a difficult subject.. It is needed not only the institutional support system but also the field mentor to promote successful experience of oral hygiene care
