조혈모세포이식을 받은 환아 어머니의 돌봄에 대한 문화기술지 연구

An Ethnographic Research on Caring of Mothers for Their Children With Stem Cell Transplantation

목적: 본 연구는 조혈모세포이식 치료를 받는 아동을 돌보는 어머니들이 공유하는 경험의 문화적 패턴을 탐색하고 그 의미를 분석함으로써 그들 집단에 대한 총체적인 이해를 제공하고자 한다. 방법: Spradley의 문화기술지 연구방법을 적용하였고, 난치성 질환으로 조혈모세포이식술을 받은 자녀를 돌본 경험이 있는 9명의 어머니를 대상으로 참여관찰, 비형식적 면담, 전화면담의 현장연구기법을 사용하여 포화수준에 이를 때 까지 자료를 수집하고 분석하였다. 결과: 어머니들의 돌봄 경험을 분석한 결과 ‘아이의 재탄생을 위한 산고의 과정’ 이라는 문화적 주제가 도출되었다. 이는 ‘이식에 대한 양가감정’, ‘이식을 향한 멀고 험난한 길’, ‘생과 사를 넘나드는 무균실 생활’, ‘이식 꼬리표가 남아 있는 아이와의 삶’으로 범주화 되었다. 결론: 조혈모세포이식치료를 받는 아동의 돌봄 제공자에 대한 깊은 이해를 제공하고 있는 본 연구결과를 바탕으로 간호실무 및 연구에 유용한 근거를 제공하는 중간범위의 실무이론 개발을 제언한다.

Purpose: This study aims to explore and understand experiences of mothers who are caring children with stem cell transplantation in the sociocultural context of Korea. Method: Using Spradley‘s ethnographic research method, this study has 9 mothers as subjects caring for their children who were treated stem cell transplantation for incurable disease. The data collection and analysis were done until the saturation point using filed research techniques such as participant observation, informal in-depth interviews, and telephone interviews. Results: The study has found a common cultural theme from the mothers’ experience expressed as ‘the process of labor for the rebirth of a child’. This can be divided into the following 4 categories: ‘ambivalence about a transplantation , ‘a long and rough road to transplantation , ‘living in a sterile room crossing life and death’, and ‘living with my child having a transplantation tag . Conclusion: Based on the results of this study, This study proposes o develop a mid-range practical theory which is useful for nursing practice and research to support mothers who are caring their children with stem cell transplantation.
