한국 해기사의 심정지 대비를 위한 응급상황관리 실태조사

A Study on Emergency Situation Management for Cardiac Arrest of Seafarers in Korea

본 연구는 해상선박에서 해기사들의 심정지 상황에 대비하기 위한 응급상황관리실태를 파악하여 향후 기초교육과 지원체계 마련의 근거자료를 제공하고자 시행되었다. 근무하는 선박 내 AED가 비치된 곳은 20%미만이었고 CPR자격증을 취득률도 50% 미만이었다. 이들의 심폐소생술 지식정도는 10.88점±3.86(총점 20점), 심폐소생술에 대한 태도는 43.38±11.02점(총점 70점), 심폐소생술 수행능력정도는 49.22±11.02점(총점 80점)으로 모두 중간보다 조금 높은 수준이었다. AED가 비치된 경우 심폐소생술 수행능력점수가 유의하게 높았으며 CPR 교육을 받은 사람은 심폐소생술 지식은 유의하게 높았으나 수행능력은 차이가 없었다. 그러나 CPR 자격증 취득자와 CPR 자신감이 있다고 답한 사람은 심폐소생술 수행능력점수가 유의하게 높았다. CPR교육이수기간이 오래되었을수록 심폐소생술 태도와 수행능력점수가 낮았다. 제한적 의료상황에서 의료관리자 해기사들이 신속하게 대처할 수 있도록 CPR교육, 자격증 취득, 기초시설관리 등 실질적인 시스템 지원이 요구된다.

This is a descriptive survey research for the purpose of finding emergency situation management status for seafarers’ cardiac arrest on the ship. The AED equipping rate on the ship was less than 20%, and CPR license acquisitor among subjects was less than 50%. Their CPR knowledge score was 10.88±3.86(total 20), attitude score for CPR was 43.38±11.02 of total 70, and performance score was 49.22±11.02 of total 80. They experienced inadequate ventilation facility in 42.6%, and 20.6% of them didn’t have separating room for smoking on the ship. As a result of analyzing difference in variables, there were statistically significant difference in CPR knowledge by CPR education experience, and CPR performance by AED equipping condition, CPR license acquisition and confidence for CPR. The older time period from the CPR education experience, the lower of CPR performance score and confidence. From these findings, seafarers will be provided with effective support system to act quickly for CPR in limiting medical condition.
