Ossifying lipomas without connection to bony structures within the intramuscular area are extremely rare. We report a case of 46 year-old man with diagnosed as intramuscular ossifying lipoma with cartilage formation located in the left gastrocnemius muscle. On T1 and T2 weighted MRI, the mass appeared that the lesion was situated at the left gastrocnemi니s muscle and showed ill-defined heterogenous mass sized 2.5 cm in longest diameter with mottled-shaped, irregular specules of calcification and ossification. He underwent mass removal. Microscopically, ill-defined mass of mature adipose tissue with trabeculae of vital lamellar bone within skeletal muscle is noted. A portion of enchondroossification is noted.
