Prognostic Factors on Poor Visual Acuity after Vitrectomy for Proliferative Diabetic Retinopathy

Purpose: This study investigated the association of the postoperative visual acuity and prognostic factors in vitrectomy for the proliferative diabetic retinopathy. Subject and Methods: Medical records of 178 eyes in 151 patients who had undergone pars plana vitrectomy for the complications of proliferative diabetic retinopathy and followed more than 3 months, were reviewed. Results: Postoperatively, visual acuity was improved in 125 eyes (70.3%), unchanged in 15 eyes (8.4%), and decreased in 38 eyes (21.4%). Preoperative iris neovascularization and intraoperative gas or silicone oil injection proved to be detrimental to visual outcome. Postoperative final visual acuity was not improved in 6 eyes of 8iris neovascularization (75%) (P=0.014) and in 13 eyes of 25 intraoperative gas or silicone oil injection (52%) (P=0.017). But there was no association between postoperative poor visual acuity with visual prognostic factors according to age, preoperative panretinal photocoagulation, preoperative visual acuity, intraoperative lens extraction and presence of diabetic nephropathy (P>0.05). Conclusion: Prognostic factors of poor postoperative visual acuity in vitrectom y for the com plications o f proliferative diabetic retinopathy were preoperative iris neovascularization and intraoperative tam ponade.
