상향 안진과 측방 주시 유발 안진이 동반된 일측성 상부연수경색 1예

A case of Upbeat Nystagmus Combined with Lateral Gaze-evoked Nystagmus in the Unilateral Rostral Medullary Infarction

2-year-old man was admitted with sudden vertigo and gait instability. He showed upbeat nystagmus at primary position, which was increased in amplitude with upward gaze and decreased with downward gaze, and horizontal nystagmus to direction of gaze during lateral gaze. His brain MRI disclosed a small paramedian infarction in the rostral medulla. This case is very unique because of special lesion localization that is helpful to understanding of the eye movement physiology. Both vertical and horizontal nystagmus may be caused by dysfunction of both vertical and horizontal neural integrators located in medulla.
