Systemic Lupus Erythematosus (SLE) is autoimmune disorder involving systemic organs and characterized commonly by fatigue, fever, myalgia, and arthralgia. Leptospirosis is a kind of Korean hemorrhagic fever disease and characterized commonly from fever, headache, myalgia severly to jaundice, renal failure, and hemorrhage. A 60-year-old woman was presented with febrile sense, myalgia, headache and general weakness. Initially, she was diagnosed as leptospirosis by serological test. During treatment her clinical manifestation was not improved. On followed laboratory data, FANA positive with leukocytopenia and hemolytic anemia. Also anti-cardiolipin antibody, anti-dsDNA antibody and proteinuria was found. The serological titer of leptospirosis was not increased by four times. So, we concluded that she had SLE and prescribed steroids. After that, her clinical manifestations were improved. Therefore, we report here on a case of SLE misdiagnosed as leptospirosis.
