특수임무수행자의 심리적 특성과 외상 후 스트레스 증상에 관한 연구: 월남전 참전군인, 일반인과의 비교

A Survey Study of Psychological Characteristics and Post-Traumatic Stress Symptoms in Veterans Who Performed Special Mission: Comparing Anti-North Secret Agents, Vietnam Veterans and Normal Control Subjects

본 연구는 특수임무수행자의 외상 후 스트레스 장애 증상 관련 변인과 심리적 증상 및 방어기제 양상을 알아보기 위해 수행되었으며, 월남전 참전군인, 일반인과의 비교를 통해 심리적 문제 수준의 차이를 파악하였다. 특수임무수행자 9명과 월남전 참전군인 14명, 일반 군 복무 경험이 있는 동 연령대의 일반인 14명을 대상으로 외상 및 복합 외상 후 스트레스 증상 척도, 외상 후 성장 척도, 단축형 간이정신진단검사, 방어유형 질문지를 실시하였으며, 수집된 자료를 분석하였다. 연구결과, 외상 및 복합 외상 후 스트레스 증상은 특수임무수행자 집단, 월남전 참전군인 집단, 통제집단 순으로 통계적으로 유의하게 높은 수준을 보이는 것으로 나타났다. 반면, 외상 후 성장에서는 집단 간 통계적으로 유의한 차이가 나타나지 않았다. 다음으로, 정신진단검사 결과에 대한 분석에서 특수임무수행자 집단, 월남전 참전군인 집단, 통제집단 순으로 통계적으로 유의하게 높은 심리적 증상 수준을 보이는 것으로 나타났다. 마지막으로, 방어유형에 대한 분석결과 특수임무수행자 집단은 월남전 참전군인 집단과 통제집단에 비해 부적응적 행동 방어 군집을 더 많이 사용하는 것으로 나타났으며, 통제집단에 비해 심상 왜곡 방어 군집을 더 많이 사용하는 것으로 나타났다. 또한 세부방어기제에서 특수임무수행자 집단은 월남전 참전군인 집단과 통제집단에 비해 행동화 방어를 더 많이 사용하는 것으로 나타났다. 본 연구의 결과는 특수 임무수행자 집단이 높은 외상 관련 증상과 심리적 증상을 호소하며, 미성숙한 방어기제를 사용하고 있음을 시사한다.

The aim of this study is to investigate post-traumatic stress and symptoms, psychological symptoms and defense mechanism style in veterans who performed special mission, and to confirm a difference in level of psychological problems among anti-north secret agents(n=9), Vietnam veterans(n=14) and normal control subjects(n=14). All participants responded to PTSD and complex PTSD symptoms scale, Brief Symptoms Inventory and Defense Style Questionnaire. Significant differences were found in PTSD and complex PTSD symptoms as well as psychological symptoms measured using BSI. All symptoms scored high in order of the anti-north secret agent group, the Vietnam veteran group, and the control group. Also, the anti-north agent group showed higher level of using immature and maladaptive defense cluster than other groups. Especially, the anti-north agent group used acting out defense more than other groups. The results of this study suggest that trauma-related symptoms, psychological symptoms and using of immature defense mechanism which were reported by anti-north agent have affected negative effect in their lives.
