Brugada syndrome is diagnosed in patients with typical ECG findings (coved ST elevation in leads V1-V3) plus at least one additional criterion from: a personal or family history of syncope, ventricular arrhythmias, or similar ECG findings in other family members. It is usually diagnosed in adulthood, and is very rarely diagnosed in children. A 15-year-old boy collapsed and lost consciousness while sitting at school. There was no significant previous history. An ECG showed ventricular fibrillation, which was successfully defibrillated with automated external defibrillator. 24-hour holter monitoring, coronary angiography, and electrophysiology tests did not show any specific findings. The flecainide-induced test showed RBBB with ST elevation in V1 to V2, suggesting Brugada syndrome type 1. He was successfully treated with implantable cardioverter defibrillator and was discharged.
