For non-extraction treatment of Class II malocclusion with crowding, maxillary molar distalization is essential to gain available space for leveling and improving dental relationships. Skeletal anchorage is beneficial to distalize the maxillary molars without anchorage loss and patient cooperation. This case report describes the distalization of the maxillary molars using palatal plate (3-dimensional key plate; 3DKP). A 21-year-old female presented with chief complaints of dental irregularity and high canine. She had a hyperdivergent skeletal pattern, Class II dental relationships and crowding but an acceptable facial profile. The posttreatment occlusion was significantly improved by maxillary molar distalization using 3DKP for leveling the dentition without any significant change on the facial harmony.
