Colorectal metastasis from gastric cancer is rarely reported and presented with various clinical and endoscopic findings. Prompt diagnosis and appropriate management are important factors in determining prognosis. A 57-year-old man who underwent a total gastrectomy with gastric cancer four years ago presented with bowel habit change. Abdominopelvic computed tomography and colonoscopy revealed diffuse luminal stricture with edematous wall thickness in the rectum without abnormalities in other areas. Histologic examination confirmed a poorly differentiated adenocarcinoma with signet ring cells that were found to be positive Cytokeratin 7 and negative Cytokeratin 20 and CDX-2. Here, we report our case of metachronous rectal metastasis from gastric cancer with a brief review of literature.
