입원 노인 환자를 위한 작업-기반 수면중재 프로그램 효과: 단일사례연구

The Effects of Occupation-Based Sleep Intervention Program (OBSP) for Inpatients in Elderly: A Single Case Study

연구목적 : 작업치료사들은 클라이언트의 수면에 영향을 미치거나 수면으로 영향을 받을 수 있는 작업과 환경, 일상에 대하여 이해하고 접근하는 전문가이지만, 이에 대한 연구가 미비한 실정이며, 임상적 접근의 기회가 없는 것이 현실이다. 작업치료 고유의 작업-기반 수면중재 프로그램의 효과성을 제시함으로써 국내 작업치료사들의 수면 관련 중재를 위한 기반을 제공하고자 한다. 연구방법 : 본 연구의 대상자는 부산광역시의 A,B,C 요양병원에 입원 중인 65세 이상의 노인 3명을 대상으로 실험은 ABAB 반전설계로 진행되었으며 총 74회기에 걸쳐 시행되었다. 연구결과 : 대상자 모두 수면효율과 수면만족도에 대한 기초선과 중재선을 확인한 결과 작업-기반 수면중재 프로그램을 수행한 대상자들에게서 수면효율과 수면 만족도에 효과가 있는 것을 확인할 수 있었다. 결론 : 작업-기반 수면중재 프로그램은 입원 노인들의 수면장애 완화에 효과적인 작업치료 중재방안임을 확인하였다. 입원 노인들의 신체적, 정신적 안정을 도모하는데 많은 도움이 되는 프로그램이라고 예상된다.

Objective : Occupational therapists are experts who understand and approach the work, environment, and daily life that may affect or be affected by a client s sleep, but the research on this is insufficient and there is no opportunity for clinical approach. By presenting the effectiveness of occupational therapy s unique occupation-based sleep intervention program, it is intended to provide a basis for sleep-related interventions of domestic occupational therapists. Methods : The subjects of this study were three elderly people aged 65 years or older who were admitted to nursing hospitals A, B, and C in Busan, and the experiment was conducted with an ABAB reversal design and was conducted over a total of 74 sessions. Results : As a result of confirming the baseline and intervention lines for both sleep efficiency and sleep satisfaction, it was found that the subjects who performed the work-based sleep intervention program had an effect on sleep efficiency and sleep satisfaction. Conclusion : It was confirmed that the occupational-based sleep intervention program is an effective occupational therapy intervention for alleviating sleep disorders in the elderly in hospitalization. It is expected to be a program that is very helpful in promoting physical and mental stability of the elderly in hospitalization.
