This case report describes the treatment of a 10-year-old male patient with a palatally impacted canine using open surgical exposure and bone-anchor pendulum. He showed moderate crowding and a palatally impacted canine in the maxillary arch and a labially impacted canine in the mandibular arch. A palatally impacted canine was subjected to surgical exposure with the open technique followed by molar distalization using a bone-anchored pendulum to regain the space and prevent anchorage loss. Comprehensive orthodontic treatment was proceeded. After the orthodontic treatment, the canine was well aligned, and favorable occlusion was achieved. Open surgical exposure allowed the palatally impacted canine to erupt autonomously to the level of the occlusal plane and provided simple, predictable, biologically sound, and esthetic outcome. The bone-anchored pendulum appliance was an effective, minimally invasive, and compliance-free intraoral appliance for achieving molar distalization without any anchorage loss.
