Deep bite is one of the challenging malocclusions to correct, and it appears as a combination of dentoalveolar and skeletal characteristics. In patients with skeletal deep bite, excessive overbite can be reduced through clockwise rotation of the mandible. However, this type of rotation may worsen the sagittal problems and smile esthetics. Therefore, in patients with deep bite and excessive gingival display, detailed analysis and treatment planning is required for aesthetically optimal results. A 20-year-old man with a chief complaint of excessive gingival display when smiling showed deep bite and skeletal Class II relationship. The patient was treated for 5 months with intrusion of maxillary incisors using two orthodontic mini-implants placed on the interdental space between the lateral incisor and canine. After obtaining a proper overbite, comprehensive orthodontic treatment was initiated and was followed by total arch distalization of the maxillary dentition to improve the canine and molar relationship. After 28 months of treatment, good occlusal relationship and satisfactory smile esthetics were achieved.
