말기 암 환자들의 필요에 대한 질적 연구

Understanding the needs of patients in palliative care

본 연구는 말기 암 환자들이 그들의 독특한 상황에서 피할 수 없이 경험하게 되는 신체적, 심리정서적, 사회적, 영적 경험들과 필요들을 고찰하고자 하였다. 연구방법으로는 반 구조화된 질문을 통한 질적 연구를 수행하였으며, 연구 참여자들은 경북지역 두 종합병원 완화치료센터에서 입원 치료 중인 환자들 중 주치의의 추천을 받은 22명의 말기 암 환자들이다. 면담을 통해 수집된 자료들의 분석 결과에 따르면, 그들의 우선된 바람은 신체적 고통을 줄이는 것이었으며, 그들이 주로 경험하는 부정적인 감정에는 미안함, 후회, 외로움, 슬픔, 서러움, 아쉬움 등이 있었고, 그들의 심리정서적 경험에 가장 크게 영향을 끼치는 것은 그들이 가지고 있는 가족들과의 관계의 질이었다. 많은 환자들이 암 진단 후에 신앙에 새로운 관심을 갖게 되거나 더 열심을 내게 되었고 그들의 신앙은 질병에 더 효과적으로 대처할 수 있도록 도움이 되었다. 본 연구 결과를 기초로 한 기독교상담 및 심리치료적 적용이 논의되었다.

This article explores the unique and complex experiences and needs of patients in palliative care. The qualitative study method was utilized to collect the data of the participants’ needs through semi–structured interviews. 22 participants were interviewed from2 palliative care units in 2 general hospitals in Kyungsang–Buk–Do. The analysis of the collected data involved the descriptive and interpretive coding and thematic content analysis. The results of the study reveal some important themes of the participants’ experiences including22 the primary painful experience of the participants is physical pain, 22 the participants desire to involve with the decision making process in their treatment, 3) the participants’ emotional experiences are rooted in their relational quality with their family members, 4) the participants are more open to spirituality, facing their death. Based on the findings, some suggests were made from Christian counseling perspective.
