알코올사용장애로 이중진단을 가진 사람의 회복 경험에 대한 생애사 연구

A Life History on the Recovery of a Person Who Had Dual Diagnosis Due to Alcohol Use Disorder

본 연구는 알코올사용장애로 인하여 이중진단을 가진 사람의 회복 경험을 생애사로 통찰하는 것을 목적으로 한다. 이를 위하여 본 연구에서는 알코올사용장애로 인한 우울, 불안 및 불면을 호소하여 정신과적 질환의 이중진단을 받은 후 17년 동안 단주를 유지해온 연구참여자 1명을 섭외하여 심층 면담을 수행하였다. 수집된 원자료는 Mandelbaum(1973)이 제안한 3가지 개념틀인 삶의 영역, 전환점, 적응으로 분석하였다. 분석결과, 연구참여자 L이 알코올사용장애로 인한 이중진단을 가지게 된 삶의 영역에는 ‘조손가족으로 어린 나이에 음주 시작’, ‘고단한 삶을 달래는 수단으로 음주’, ‘음주 폐해로 일상생활이 어려워짐’, ‘알코올로 인한 우울증이 깊어짐’이 주요 주제로 도출되었다. 삶의 전환점에는 ‘인간다운 대우를 받고 신앙의 길로 들어섬’, ‘삶의 은인들을 만나 변화가 시작됨’, ‘하느님의 손길로 회복이 시작됨’이 주요 주제로 도출되었다. 17년 동안 단주를 유지할 수 있었던 적응 전략으로는 ‘여전한 음주 문제에도 삶의 의지를 놓지 않음’, ‘소속감과 보람으로 사는 사회적 존재’, ‘회복을 위한 연대 노력’이 주요 주제로 도출되었다. 결론에서는 알코올사용장애로 이중진단을 가진 정신장애인의 회복을 위해 삶의 조력자들, 병원 치료, 주거 지원, 영적 안정감, 단주모임, 안정된 일자리, 기초생활수급 지원 등 지역사회와 국가 제도 전반의 연대적 노력이 중요함을 논하였다.

The purpose of this study was to gain an insight into a lifetime of recovery experience of a person who had dual diagnosis such as depression, anxiety, and insomnia due to alcohol use disorder. To this end, this study conducted an in-depth interview with one research participant who had maintained abstinence for 17 years after receiving dual diagnosis due to alcohol use disorder. The collected raw data were analyzed in three conceptual frameworks proposed by Mandelbaum(1973): dimension of life, turning points and adaptation. The results of this study are as follows: the dimension of life appeared as ‘start drinking at a young age as a grandchild’, ‘drinking as a means to comfort a hard life’, ‘drinking alcohol makes daily life difficult’, and ‘deep depression caused by alcohol.’ The turning points were revealed as ‘entering the path of faith after being treated fairly at work’, ‘encountering with life savers’, and ‘being restored by God’s touch.’ The adaptation strategies were derived from ‘holding onto the will to live despite drinking problems’, ‘living as a social being through belonging and rewarding’ and ‘joint efforts for recovery.’ In conclusion, the authors discussed the importance of community-wide joint efforts such as supporters, hospital treatment, housing support, spiritual well-being, A.A. meeting, vocational activities and cost-of-living allowance for the recovery of mentally disabled people with dual diagnosis due to alcohol use disorder.
