노인의 연명의료 태도에 영향을 미치는 요인에 관한 연구

A Study on the Factors Affecting the Attitudes of the Elderly toward Life-sustaining Treatment

본 연구는 성인 자녀와의 사회적 지원 교환과 친밀감 수준이 노인들의 연명의료에 대한 태도에 미치는 영향을 분석하는 것을 목적으로 한다. 이를 위해 2020 노인실태조사(보건복지부) 자료를 활용하였으며 비동거자녀가 있는 9,292명의 노인의 응답이 분석에 활용되었다. 본 연구는 연명의료에 대한 태도를 종속변수로 하여 인구사회학적 특성과 자녀와의 사회적 지원 교환 -정서적 지원 교환, 도구적 지원 교환, 돌봄 지원 교환, 경제적 지원 교환-, 자녀와의 친밀감- 자녀와의 왕래빈도, 연락빈도, 관계만족도-로 측정하였고 위계적 회귀분석을 적용하여 분석하였다. 연구결과 자녀에게 비의존적인 노인일수록 연명의료에 대해 반대하는 것으로 나타났고, 왕래빈도나 연락빈도보다는 자녀와의 관계만족도가 연명의료에 대한 태도에 유의미한 영향을 미치는 것으로 나타났는데, 자녀와의 관계만족도가 높을수록 연명의료를 선택할 가능성은 낮아졌다. 분석결과를 토대로 노인의 연명의료에 대한 자기결정권이 존중될 수 있도록 하는 정책 및 실천적 제안을 하였다.

The purpose of this study is to analyze the effect of intergenerational support and the level of intimacy with adult children on the attitude toward life-sustaining treatment of the elderly. For this purpose data from the 2020 Senior Citizen Survey was used, and the responses of 9,292 elderly people who did not live with their adult children were used for the analysis. This study was analyzed by applying hierarchical regression analysis. As a result of the study, it was found that older people who were not dependent on their children were more opposed to life-sustaining treatment, and that relationship satisfaction with children had a significant effect on attitude toward life-sustaining treatment, rather than frequency of visits or contact. And the higher the relationship satisfaction with children, the lower the likelihood of choosing life-sustaining treatment. Based on the analysis results, policy and practical proposals were suggested so that the elderly’s right to self-determination of life-sustaining treatment could be respected.
