틱 증상을 지닌 아동의 모래놀이 사례연구

A Case Study on Sandplay Therapy for a Child with a Tic Disorder

본 연구는 태내 9개월에 부를 잃고 공황장애를 앓는 모의 양육환경에서 욕구가 억압되어 틱증상이 나타나는 초등학교 5학년 남학생의 모래놀이 사례연구이다. 내담자의 심리적 과제는 모성에 가두어져 있는 상태(감옥, 울타리)의 심약한 자아에서 탈피하고 자아를 강화하여 독립적인 남성으로 거듭나는 아동기 통과의례를 가설로 모래놀이상담을 진행하였다. 상담은 5개월간 주1회(50분), 16회기에 걸쳐 이루어졌다. 모래놀이상담을 하는 동안 치료자는 모래놀이치료실과 모래상자에서 일어나는 것을 수용하고 담아주는 심리적 용기가 되어 내담자가 자기 내면과 마주하는 여정에 참여했다. 전체 여정을 살펴본 결과, 내담자의 모래놀이는 아동기 자아발달의 일반적인 과정인 혼돈, 투쟁, 집단 적응의 세 단계에 걸쳐 진행된 것으로 판단된다. 내담자는 모래놀이가 가진 치유적 힘을 통해 자아를 강화하여 모성으로부터 점차 분리하고 남성성을 키워 집단에 적응해 나간 것으로 보인다. 더불어 이 연구는 자유롭고 보호된 공간을 제공하는 모래놀이상담이 심약한 아동의 자아강화와 틱의 완화에 효과적인 매체임을 확인한 데 의의가 있다.

This study presents the case of sandplay therapy for a 5th grade boy, who had a tic disorder due to his suppressed desires resulting from his family environment of deceased father in 9 month of pregnancy and mother suffering panic disorder. Sandplay therapy was conducted based on the hypothesis that one’s psychological task is to go through a rite of passage to become an independent man by breaking free from the confined state (prison, fence) of the ego. Counseling was held once a week (50 minutes) for 5 months over a total of 16 sessions. During the sandplay therapy, the therapist became psychological container to embrace what happens in the sandbox and the sand therapy room to accompany the client’s journey to find himself. After looking at the whole journey, the sandplay of the client was divided into the three phases, chaos, fighting and group adaptation, which are the general processes of childhood rite of passage. It appears that the client separated himself from the attached mother (confining mother) through the healing powers of sandplay, trained and reestablished his ego to be reborn as a masculine individual being. Furthermore this study shows its meaning in confirming that the sandplay therapy, which provides free, safe and protected space can be effective in strengthening one’s ego and improving tic disorder.
