혈액투석 환자의 증상 클러스터가 자기효능감에 미치는 영향

Factors of Symptom Cluster Associated with Self-Efficacy in Patients with Hemodialysis

Purpose: This study was to identify the symptom clusters of patients with hemodialysis and to determine their effect on self-efficacy. Methods: Data were collected from 158 patients who were diagnosed with chronic renal failure and were undergoing hemodialysis in hemodialysis units. A questionnaire was used as a research tool, and the SPSS 27.0 program was used for statistical analysis. Results: Among the hemodialysis-related symptoms of the subjects, the most frequently complained of was ‘fatigue or lack of strength’, and ‘numbness and tingling in the feet’ were the most severe symptoms. Six symptom clusters were extracted: emotional, sensory-pain, uremic syndrome, skin, sex, and gastro-intestinal clusters. Factor affecting self-efficacy was identified as monthly income status (≥300) (=.20, p =.016)and sensory pain cluster (=−.24, p =.027). Conclusion: Nursing intervention for patients with hemodialysis requires a symptom cluster approach in which several symptoms appear simultaneously rather than an individual symptom approach. In particular, interventions for sensory-pain clusters are needed to enhance their self-efficacy, and customized symptom management strategies should be provided by identifying common mechanisms and causes of symptoms in each cluster.
